Transition Age Youth Mental Health Project is a free, confidential program designed for individuals who are clients of the San Diego Regional Center in Imperial County and who are not effectively accessing mental health services at school districts.

Our goal is to work in partnership with you to provide connections to mental health and substance use support, provide strategies to change/reduce behaviors that are causing you challenges in school; accompany you and family to IEP meetings; conduct outreach activities and training with schools and the community. Who is eligible for Transition Age Youth Project?

  • Individuals ages 14 to 22,
  • Live in the Imperial Valley,
  • Are served by the San Diego Regional Center, and
  • Have a dual diagnosis or are at risk of developing a mental health condition but are not effectively accessing mental health services or treatment.

What can you expect?

Upon joining our program, you will be partnered with a Peer Behavior Interventionist, who has personal lived experience with behavioral challenges. Peer Behavior Interventionist will focus on serving transition age youth ages 14-22 who are not effectively accessing mental health services at school districts for any reason including diagnostic over-shadowing and will offer support to your family too!  They will meet with you regularly in the community, as well as be available via phone/text.

Our Team!

Peer Behavior Interventionist, Master’s Level Clinician, and Program Manager are bilingual and have extensive experience working with participants of diverse ages and backgrounds. And, they have extensive experience serving individuals and families in Imperial County!
