Family Voice meetings are for individuals looking for more information and resources on a variety of topics. Each meeting covers a different topic with experts in the arena presenting and answering questions in an engaging manner so that all feel answered and included.
Previous topics and presenters have included; The L.E.A.P. Method, The Care ACT, informational sessions about various NAMI apps such as alfrEDU and PERT/MCRT just to name a few.
Cal-Aim and Enhanced Care Management
Please join us on December 2nd at 2:00 PM for an engaging and informative discussion on Cal-Aim and Enhanced Care Management.
Enhanced Care Management is a free Medi-Cal benefit that offers intensive care coordination for individuals with complex physical, behavioral, and social needs. Eligible members are paired with a Lead Care Manager who provides hands-on support and meets clients wherever they are. That could be in shelters, hospitals, or at home. ECM is available to Medi-Cal members experiencing challenges like homelessness, serious mental illness, or frequent hospital visits, and requires active Medi-Cal through Community Health Group, Molina, or Blue Shield. Community Supports complement ECM by addressing social needs like housing instability, with services such as housing navigation, move-in assistance, and tenancy support. Together, ECM and CS help individuals stabilize their lives and access the care and resources they need.
