POSITION TITLE:  HOMELESS OUTREACH WORKER COORDINATOR

Primary Responsibilities/Function: The Homeless Outreach Worker Coordinator will provide mobile, street and field-based outreach services in the Central Region of San Diego County and will provide coordination and support to outreach workers at the NAMI San Diego’s Connection 2 Community Club House.  The ideal candidate facilitates a positive team dynamic, is open to feedback from team members, and can engage in support of team mates.  They will be able to train others by example of utilizing an integrated care approach.  The Homeless Outreach Worker Coordinator will provide both outreach/engagement services and brief case management interventions to quickly assess and link homeless individuals to the NAMI San Diego Clubhouse, where individuals will be offered membership and assistance to accessing services. The Homeless Outreach Worker Coordinator will work as part of the NAMI SD Mobile Navigation Team and other service providers to identify homeless trends and work closely with the community to identify solutions.

Duties and Responsibilities:  

  • Coordinate and support Homeless Outreach Teams for NAMI San Diego’s Connection 2 Community Clubhouse.
  • Works long side program manager to provide supervision for the Homeless Outreach teams for NAMI San Diego’s Connection 2 Community Clubhouse.
  • Engage interviews to acquire new staff for the clubhouse.
  • Facilitate trainings and support of outreach staff.
  • Create and implement schedules for outreach coverage.
  • Insure that data entry deliverables are met by the Outreach teams.
  • Conduct mobile/street outreach to develop a trusted relationship, build rapport that will assist homeless individuals with addressing barriers to obtain housing, services and appropriate health care needs.
  • Link and provide transportation to homeless individuals to the NAMI San Diego Clubhouse; educate on membership and the International Clubhouse model. Enroll to membership as the homeless individual indicates interest in becoming a member.
  • Meet with active individuals/members to review evaluate, and support completion of their goals.
  • Conduct a VI-SPDAT (Vulnerability Index-Service Prioritization Decisions Assistance Tool) on all new members to connect them to the proper intervention and services. Enter results into HMIS in a timely manner.
  • Assist the member with benefits establishments, obtaining a birth certificate, social security card, housing referrals, and health related assistance.
  • Work collaboratively with local law enforcement and services providers to ensure transition from streets into interim housing is collaborative and meeting members and community’s needs.
  • Ensures and oversees mobile navigation vehicle maintenance to manufactures warranty and instructions. Including taking care of regular oil, gas fill-up and mileage logs.
  • As advocates for caseload, will provide case management services and advocacy within the Clubhouse and the community.

Range of Authority: The Homeless Outreach Worker Coordinator reports to the Program Manager.  The Homeless Outreach Worker Coordinator supervises the NAMI San Diego’s Connection 2 Community Club House Outreach teams.

Minimum Requirements, Education, Experiences, Licenses and Certifications:   

  • Bachelor’s Degree in human resource field and one year of working with homeless, mental illness, substance use disorder or with vulnerable populations or equivalency in career expertise.
  • Experience in Homeless Outreach
  • Experience in management preferred
  • Computer proficient in Microsoft Office and familiar with Service Point- HMIS and Club HOMS preferred.
  • Fluency in English and additional language preferred.
  • Preferred lived experience in mental health of self

COVID Protocols and Work Environment.  As an essential worker providing both telehealth and in-person services, successful candidate will follow CDC guidelines, state guidelines and local mandates for COVID 19 prevention.  COVID-19 Vaccination required for employment.

Full-time – 40 Hours                      Hourly Salary Range: $22.50
