Coordinated Family Support (CFS) is a free, confidential program designed for 18 years and older) who live with their family in the Imperial County and who are clients of the San Diego Regional Center in Imperial County and who are not effectively familiar with community resources.

Coordinated Family Support (CFS) providers will prepare a service plan designed to help reduce or remove barriers to staying in the family home, address challenges and meet goals. Services will be provided in a manner that respects the language & culture of individuals and their families, and services will be tailored, individualized, and flexible to meet the changing needs and preferences of individuals.

Who is eligible for Coordinated Family Support?

  • 18 years and older) who live with their family in the Imperial County.

What can you expect?

Upon joining our program, you will be partnered with a Peer Behavior Interventionist, who has personal lived experience with behavioral challenges. Peer Behavior Interventionist will help by developing skills at home by coordinating and obtaining services, including help with “generic services” and helping people access their community. They will meet with you regularly in the community, as well as be available via phone/text.

Peer Behavior Interventionists, Master’s Level Clinician, and Program Manager have extensive experience working with participants of diverse ages and backgrounds. They have extensive experience serving individuals and families in Imperial County!
